Qualifications

• Active Registered Nurse (RN) license in the state you work (PA).
• Two years of RN experience; Hospice experience preferred.
• Driver’s license required; ability to travel ~20% of time.

Responsibilities

• Deliver patient-centered hospice care in patients' homes as part of an interdisciplinary team.
• Perform comprehensive patient assessments, symptom management, and end‑of‑life care planning.
• Educate and support patients and families; ensure compliant EMR documentation.
• Travel to patient homes within assigned territory; work independently yet within a team.
