Graham Healthcare Group seeks a Registered Nurse (RN) to join its Hospice team in Hershey, Pennsylvania. This staff position delivers compassionate end-of-life care in patients’ homes, with a 40-hour work week and 8-hour shifts, traveling within the assigned territory.

Qualifications

• Active Registered Nurse license in the state you work
• Two years of RN experience and the clinical expertise to deliver quality patient care; Hospice experience preferred
• Current driver’s license and ability to spend ~20% of your day driving to/from patient locations

Responsibilities

• Deliver compassionate, patient-centered care in a hospice setting, collaborating with an interdisciplinary team dedicated to comfort, dignity, and quality of life
• Perform comprehensive patient assessments, symptom and pain management, and individualized end-of-life care planning
• Educate and support patients and families through the end-of-life journey, ensuring they feel informed, prepared, and deeply cared for
• Think critically, solve problems in real time, and ensure thorough, compliant EMR documentation
• Work independently, but never alone
• Travel to patient homes within assigned territory
