Qualifications

• At least 2 years of RN experience, preferably in hospice.
• Current driver's license and ability to spend ~20% of your day driving.
• Commitment to meeting critical deadlines for charting.

Responsibilities

• Collaborate with the care team to manage and deliver patient care plans.
• Support patients in Moosic, PA, and surrounding areas.
• Engage in a team-based care environment.
