Job summary

Suncrest Hospice in Ohio is seeking an experienced Registered Nurse (RN) to provide compassionate care to a group of patients in their homes or care facilities. You will work as part of an inter-disciplinary team, delivering psych-social assessments and coordinating with other healthcare professionals to support patient comfort and quality of life.

You must hold an active RN license in good standing, thrive in a collaborative environment, and be willing to travel within the service area.

Qualifications

• RN license in good standing required.
• Ability to collaborate daily with nurses, social workers, chaplains, volunteers, and CNA leadership.
• Strong time management to schedule visits, documentation, and meetings.
• Willingness to drive within the service area with reliable transportation.

Responsibilities

• Provide care and services for a group of patients in homes, SNFs, and assisted living facilities.
• Contribute to the interdisciplinary team with psych-social assessments and community resource knowledge.
• Possibility to serve in Case Management, Admissions, or Resource roles.

Education
