Job summary

Interim HealthCare Hospice seeks a Nursing RN to provide compassionate home-based care to patients with limited life expectancy. You will collaborate with physicians, LPNs, CNAs, and social workers to implement the plan of care and support families during hospice.

The role requires an active Michigan RN license, at least 1 year of RN experience, and reliable transportation. Join a nationwide network that prioritizes patient comfort and dignity.

Qualifications

• RN with active, unrestricted license in the State of Michigan.
• At least one year of RN experience.
• Experience in Community health and Medical-Surgical settings preferred.
• Reliable transportation with a valid state driver's license and current car insurance.

Responsibilities

• Provide home-based care to patients with six months or less to live.
• Work as part of an interdisciplinary team to carry out the patient's plan of care.
• Perform patient assessments, check vitals, evaluate breathing, monitor pain, administer prescribed medications.
• Educate family on prognosis, changes in care plan, and hospice goals.
• Coordinate with medical equipment, oxygen, and supplies as needed.
