Job summary

Interim HealthCare is seeking a dedicated Hospice RN Case Manager in Michigan. You will provide skilled home-based nursing care, oversee a team of LPNs, CNAs, and aides, and coordinate patient care from admission to discharge.

Qualifications

• Active RN license in Michigan with unrestricted status.
• At least 1 year of clinical RN experience; hospice experience preferred.
• Current CPR certification and reliable transportation are required.

Responsibilities

• Provide home-based nursing care to patients in their homes.
• Oversee LPNs, CNAs, and aides delivering patient care.
• Assess patients, document progress, and report changes to physicians.
• Educate patients and families on care plans, medications, diet, and safety.
• Coordinate discharge planning and ensure goals are met.
