Job summary
Interim HealthCare is seeking a Hospice RN Case Manager in Michigan to provide home-based nursing care and coordinate with physicians. Qualifications
- Active, unrestricted RN license in Michigan.
- At least 1 year of RN clinical experience (Hospice preferred).
- Current CPR certification is required.
- Reliable transportation with a valid driver’s license and car insurance.
Responsibilities
- Provide home-based care to patients in their homes.
- Oversee LPNs, CNAs and aides delivering patient care.
- Examine patients, perform assessments, document progress, report changes.
- Educate patients and families on care plans, medications, and safety.
- Coordinate discharge planning and follow-up as needed.
