Job summary

BAYADA Home Health Care is seeking a full-time Registered Nurse (RN) to join our Hospice team in Concord, NH. The RN will provide compassionate 1:1 care to hospice clients, including medication administration, wound care, and symptom management in patients’ homes.

Qualifications

• Current NH RN license required.
• Minimum 1 year of recent clinical experience.
• Home care and/or hospice experience preferred.
• Ability to travel to assigned cases.

Responsibilities

• Make home visits to clients in designated territories.
• Administer medications, perform wound care and treatments.
• Monitor client conditions and report changes to supervisor.
• Document doctors' orders accurately.
• Conduct client assessments as needed.
• Occasional on-call after hours and weekends.
