Job summary

BAYADA Home Health Care is seeking a Registered Nurse (RN) Case Manager, Hospice for a nursing role in Cornish, New Hampshire. This full-time position involves in-home visits and coordinating care for hospice clients within NH/VT service areas.

The role requires an active RN license in VT and NH, strong clinical skills, and the ability to travel. Comprehensive BAYADA benefits and opportunities for growth are provided.

Qualifications

Current RN license in VT and NH.
Minimum one year of clinical (medical/surgical) experience.
Home care and/or hospice experience preferred but not required.
Solid computer skills; EMR experience preferred.

Responsibilities

Make home visits to clients in designated geographic territories.
Administer medications, wound care, treatments, and comfort care.
Monitor clients' conditions and report changes to the Clinical or Client Services Manager.
Follow up with, execute, and document doctors' orders.
Perform client assessments as necessary.
Occasional on-call after hours and/or weekends.
