Job summary

Qualifications

• Current RN license in VT and NH.
• At least 1 year of recent clinical (medical/surgical) experience.
• Home care or hospice experience preferred.
• Strong computer skills; EMR experience preferred.

Responsibilities

• Make home visits to clients in designated geographic territories.
• Administer medications, perform wound care and comfort measures.
• Monitor client conditions and report changes to supervisor.
• Document doctors' orders and complete required follow-up.
• Occasional on-call after hours and/or weekends.
