Qualifications

• WA RN license required within hire, Vendor Managed.
• BLS from American Heart Association required within 30 days of hire.
• Associate’s or Bachelor’s degree in nursing or equivalent experience.

Responsibilities

• Completes comprehensive assessments of patients and families to determine hospice needs.
• Develops Plans of Care in collaboration with the IDG and sets goals.
• Provides nursing care, administers medications, and counsels patients and families.
• Coordinates with the IDG for continuity of care and safety.
• Maintains clinical documentation and responds to changes in patient condition.
• Participates in on‑call duties and supports team education and development.
