Job summary

Providence at Home with Compassus is seeking a Hospice Registered Nurse (RN) to deliver compassionate, person-centered care in patients' homes. You will assess needs, develop and direct plans of care with the Interdisciplinary Group, and support families with empathy and clinical excellence.

The role emphasizes nursing process, education, and collaboration; requires an active RN license and BLS within 30 days of hire. This is a full-time, day-shift position with competitive pay and benefits.

Qualifications

• Associate or Bachelor's degree from an accredited School of Nursing.
• 1 year experience practicing as a registered nurse preferred.
• Current, unencumbered RN license.

Responsibilities

• Completes assessment of patient and family to determine Hospice needs.
• Provides a complete physical assessment and history and provides professional nursing care by utilizing all elements of the nursing process.
• Writing and initiating Plan of Care in collaboration with the IDG.
• Regularly reevaluating patient/family/caregiver needs and revising the Plan of Care as necessary.
• Initiates the Plan of Care and makes necessary revisions as patient status and needs change.
• Develops a care plan in conjunction with the patient/family/caregiver and IDG which establishes goals based on nursing diagnosis and incorporates palliative nursing actions.
• May administer medications and treatments as prescribed by the IDG approved Plan of Care.
• Participates in on-call duties as defined by the on-call procedures.
• Maintains productivity standards as set by organization guidelines.
• Documentation/Information Management: Completes, maintains and submits accurate and relevant clinical notes regarding patient’s condition and care given.
• Supervises ancillary personnel and delegates responsibilities when required.
• Participates in orientation, education, and development of team members.
• Assumes responsibility for personal growth and development and maintains and upgrades professional knowledge and practice skills through attendance and participation in continuing education and in-service classes.
