Job summary

Providence at Home with Compassus seeks a Registered Nurse to plan, organize and direct hospice care in home environments. Experience in nursing with emphasis on community health education is valued; duties include collaborating with the IDG and providing patient/family education.

The role covers a wide age range and requires comprehensive assessments, care planning, documentation, and on‑call duties. State RN license is required and a strong focus on compassionate home-based care is essential.

Qualifications

• RN license active and in good standing; required within state regulations.
• Eligible for RN licensure in WA/AK/OR/CA with relevant hospice/home health experience.
• Ability to perform nursing assessments and develop plans of care.
• Experience in hospice, home health, or skilled nursing preferred.

Responsibilities

• Completes comprehensive patient and family assessments to determine hospice needs.
• Develops and revises individualized plans of care with the IDG.
• Delivers professional nursing care using the nursing process and palliative interventions.
• Provides patient/family education and coordinates with IDG for continuity of care.
• Maintains timely clinical documentation of patient condition and outcomes.
• Communicates with IDG and supervisors; attends IDG meetings to review plans.
• Performs on‑call duties and maintains productivity per guidelines.
• Supervises ancillary staff and supports team development and training.
• Engages in professional growth and performance improvement initiatives.
