Pioneer Valley Hospice & Palliative Care is seeking a part-time on-call Registered Nurse to provide comprehensive hospice care in Massachusetts. The RN will triage, coordinate, and administer hospice services, ensuring patient comfort and family support.

The role requires 1–2 years of healthcare experience, MA RN licensure, and CPR certification, with preferred BSN. Evening and weekend shifts operate under a supportive team environment.

Qualifications

• Minimum of one to two years of experience in health care.
• Experience in a hospice or home health preferred.
• Current Massachusetts RN licensure.
• CPR certification required.

Responsabilités

• Demonstrates understanding of hospice philosophy and can articulate it to others.
• Manages plan of care from referral through discharge for assigned patients.
• Completes initial nursing assessment of patient/family to determine hospice needs.
• Provides complete physical assessment and history of current and previous illness.
• Initiates and implements plan of care with patient/family to promote comfort and autonomy.
• Administers medications and treatments as prescribed; documents responses.
• Collaborates with the interdisciplinary team for symptom management.”
• Coordinates patient care with team, family, and physician.
• Documents all clinical notes and measurable outcomes of care.
• Completes patient discharges and communicates per regulations.
• Communicates with medical director and attending physician as needed.
