Job summary

UMass Memorial Health is seeking a Hospice RN Per Diem to provide direct nursing care and case coordination across a 7-day week, including day, evening, night, weekend, and holiday duty. The role emphasizes safe patient environments and holistic care within the agency’s standards.

The position requires MA RN licensure, a valid driver’s license, and the ability to work autonomously with strong communication skills. A signing bonus may be available, with details confirmed during interview.

Qualifications

• Diploma or Associate’s Degree in Nursing.
• Valid and unrestricted licensure as a registered nurse in the Commonwealth of Massachusetts.
• Valid and unrestricted driver’s license recognized by the Commonwealth of Massachusetts and access to a reliable, insured vehicle.

Responsibilities

• Assures case coordination and provides direct nursing care to patients and families on a 7 day per week basis.
• Meets patient care productivity standards established by the agency.
• Collects comprehensive data pertinent to the patient’s health or the situation.
• Analyzes assessment data to determine the diagnoses or issues.
• Identifies expected outcomes in a plan individualized to the patient, family and/or caregivers and the situation.
• Develops a plan that prescribes strategies and alternatives to attain expected outcomes.
• Implement the individualized patient plan of care.
• Coordinates care delivery.
• Employed strategies to promote health, comfort, and a safe environment.
• Evaluates progress towards attainment of outcomes.
