Job summary
Autumn Hospice is seeking an RN On-Call Nurse for weekend shifts in the Norwell, MA area. The role involves delivering comprehensive hospice nursing care, performing on-call visits, and coordinating with physicians and the interdisciplinary team to ensure high-quality patient outcomes.
The ideal candidate holds an Associate Degree in Nursing, a current state RN license, and hospice or home health experience.
Qualifications
- Hospice or home health setting experience preferred.
- Current State RN license required.
- Associate degree in nursing or equivalent required.
- Excellent documentation and assessment skills.
Responsibilities
- Provide hospice nursing care through assessment, planning, implementation, and evaluation of patient care.
- Perform on-call visits and respond to patient needs during weekend shifts.
- Assess and manage pain and symptoms with compassionate end-of-life care.
- Coordinate with physicians and interdisciplinary team to plan patient care.
- Complete comprehensive patient assessments to determine hospice eligibility and needs.
- Document timely and accurate clinical information per policies and Medicare regulations.
