Pioneer Valley Hospice & Palliative Care in Massachusetts is seeking a Hospice RN for after-hours on-call coverage, 4:30 p.m.–8:00 p.m., part-time. This role involves triage, care coordination, and delivering hospice services to patients and families while coordinating with the interdisciplinary team.

Formación

• Minimum 1–2 years of health care experience.
• Hospice or home health experience preferred.
• Current Massachusetts RN license and CPR certification.
• Associate's degree in nursing required; bachelor's preferred.

Responsabilidades

• Triage, coordinate, and administer hospice care per plan of care.
• Manage plan of care from referral through discharge.
• Perform initial nursing assessment of patient/family.
• Administer medications and monitor responses.
• Collaborate with IDT to educate patient/family on pain and symptom management.
• Document clinical notes and measurable outcomes.
