Job summary

Suncrest Hospice is seeking a dedicated Registered Nurse (RN) to provide compassionate hospice care to patients in their homes or facilities across the service area. You will participate in an interdisciplinary team, offering psych-social support, care planning, and coordination of services.

The role may involve Case Management, Admissions, or Resource coordination, requiring strong collaboration with nurses, social workers, and other professionals.

Qualifications

• RN license in good standing in Texas
• Collaborates daily with nurses, social workers, chaplains, volunteers and CNA leadership
• Manages visits, documentation, meetings and urgent needs with prioritization and triage
• Familiarity with hospice rules and regulations and ability to travel across the service area

Responsibilities

• Provide care and services for a group of patients in homes, SNFs, and assisted living facilities
• Serve as a trusted member of the interdisciplinary team with psych-social assessments and interventions
• Roles may include Case Management, Admissions, or Resource coordination
