Job summary

Suncrest Hospice is seeking a Registered Nurse to provide direct patient care in home and facility settings across the service area. You will join an interdisciplinary team to coordinate psych-social assessments, hospice planning, and patient advocacy.

This role supports Case Management, Admissions, or Resource functions, requiring strong communication, travel readiness, and robust EMR proficiency to ensure high-quality care and documentation.

Qualifications

• Current RN licensure (in good standing).
• Strong teamwork and communication with a diverse care team.
• Ability to travel across the service area with reliable transportation.
• Familiarity with hospice rules and regulations.
• Proficiency with electronic medical records.
• Excellent time management and prioritization.

Responsibilities

• Provide care for a group of patients in their homes, SNFs, and assisted living facilities.
• Work as part of the interdisciplinary team in Case Management, Admissions, or Resource roles.
• Document visits and coordinate with other healthcare professionals.
