VitalCaring in Pryor, Oklahoma, is seeking a Hospice Registered Nurse (RN) to lead compassionate, patient-centered end-of-life care in field-based settings. You will manage symptoms, develop plans of care, and partner with families to honor goals and wishes.

As a clinical leader and advocate, you will coordinate with physicians, social workers, chaplains, and aides, delivering comfort, dignity, and tailored care across home and facility settings.

Qualifications

• Active RN license or compact eligibility.
• Graduate of an accredited nursing program.
• Minimum 1 year of clinical nursing experience.
• Strong clinical judgment and compassionate care approach.
• Ability to manage a caseload independently.
• Excellent communication skills in sensitive situations.
• Basic EMR proficiency.
• Valid driver’s license and reliable transportation.

Responsibilities

• Lead Patient-Centered Hospice Care: conduct comprehensive patient and family assessments, develop and manage individualized, comfort-focused care plans, deliver skilled nursing care with focus on pain and symptom management.
• Serve as the Care Coordinator: act as case manager, coordinating care across the interdisciplinary team, collaborate with physicians, social workers, chaplains, and aides, monitor patient condition and adjust care plans to maintain comfort.
• Support Patients & Families: provide education, emotional support, and guidance, help families navigate decisions with empathy and clarity, build trusting relationships.
• Drive Quality & Compliance: maintain accurate documentation, participate in IDT meetings and care planning, support quality and compliance standards.
