Qualifications

• Active RN license or compact eligibility.
• Graduate of an accredited nursing program.
• Minimum 1 year of clinical nursing experience; hospice/home health preferred.

Responsibilities

• Lead patient-centered hospice care through comprehensive assessments and comfort-focused plans.
• Coordinate care across the interdisciplinary team as care manager.
• Educate and support patients and families; navigate complex decisions with empathy.
