Job summary

VitalCaring Group in Weatherford, TX is seeking a Hospice Registered Nurse (RN) to lead and deliver patient-centered end-of-life care in home and facility settings. You will assess needs, develop care plans, manage symptoms, and advocate for patients and families with compassion and clinical excellence.

This field-based role offers autonomy with strong interdisciplinary support through physicians, social workers, chaplains, and aides.

Qualifications

• Active RN license (or compact eligibility).
• Graduate of an accredited nursing program.
• Minimum 1 year of clinical nursing experience.
• Strong clinical judgment and compassionate care approach.
• Ability to manage a caseload independently.
• Excellent communication skills in sensitive situations.
• Basic EMR proficiency.
• Valid driver’s license and reliable transportation.

Responsibilities

• Lead patient-centered hospice care with comprehensive assessments.
• Develop and manage individualized, comfort-focused care plans.
• Deliver skilled nursing care focusing on pain and symptom management.
• Act as case manager coordinating care across the interdisciplinary team.
• Collaborate with physicians, social workers, chaplains, and aides.
• Monitor patient condition and adjust plans to maintain comfort.
• Provide education and guidance to patients and families throughout the journey.
• Maintain accurate, timely documentation and participate in care planning.
