Qualifications

• Must hold a Louisiana LPN license.
• 2–3 years of LPN experience.
• Current CPR certification required.
• Reliable transportation, valid driver's license and car insurance.
• Hospice experience preferred.

Responsibilities

• Provide nursing and supportive care per Interdisciplinary Plan of Care under RN supervision.
• Communicate with physicians, hospice team, and agencies to coordinate patient care.
• Document all services on appropriate forms.
• Demonstrate knowledge of Medicare/Medicaid hospice benefits.
• Report changes in patient condition to RN.
• Manage time effectively.
