Qualifications

• Minimum of two years full-time experience as an RN in hospice, oncology, acute care, long-term care, or home health care.
• Current automobile insurance and valid driver’s license.

Responsibilities

• Perform initial and ongoing assessments of patients.
• Assist in developing and implementing the Hospice Plan of Care.
• Educate patients and families on medication administration and care.
• Act as primary liaison between physician, patient/family, and hospice team.
• Maintain accurate and updated medical records.
