Carle Health Home Services seeks a Hospice RN to join the interdisciplinary team, coordinating and delivering direct and indirect patient care across the service area. You will use the nursing process to meet physical, psychosocial, environmental, and spiritual needs of patients and families; hours vary with census and program needs.

The role requires an Illinois RN license, BLS, auto insurance proof, driver's license, and a nursing diploma with at least 1 year of nursing experience.

Qualifications

• RN license in Illinois (IDFPR).
• BLS certification within 30 days (AHA).
• Auto insurance proof and valid driver's license.
• College diploma in nursing; 1 year nursing experience.

Responsabilités

• Demonstrates understanding of Hospice regulatory requirements and documentation standards.
• Performs case management activities including assessment, coordination, planning, monitoring, and evaluation.
• Interacts with clients, caregivers and families to assess, plan care, arrange services, monitor, and educate.
• Completes admission documentation within policy timelines and ensures orders are written timely.
• Acts as coordinator of the health care team and initiates case conferencing when necessary.
