Qualifications

Responsibilities

• Assess, monitor, and document patient’s symptoms.
• Collaborate with the interdisciplinary team to develop care plans.
• Serve as patient advocate, providing information on rights and responsibilities.
• Observe and evaluate patient’s status and education needs for caregivers.
• Supervise aides and licensed practical nurses.
• Participate in on-call rotating schedule.
