HighPoint Hospice, part of the LHC Group and Optum family, is seeking a Registered Nurse Case Manager to deliver and coordinate hospice care. You will evaluate and update the Plan of Care, collaborate with a care team, and support patients and families in a home setting.

Requirements include an active RN license and a valid driver's license; CPR certification is preferred. The salary ranges from $74,880 to $83,200 annually, with a comprehensive benefits package offered for full-time employment.

Formación

• Current and unrestricted RN licensure in the state of practice.
• Current driver's license, valid vehicle insurance, and access to a dependable vehicle, or public transportation.

Responsabilidades

• Perform initial nursing evaluation to determine hospice eligibility within 48 hours of referral.
• Re-assess patient needs every 15 days and adjust plans accordingly.
• Initiate and coordinate the plan of care with the IDG.
• Document problems, goals, interventions, and patient/family responses.
• Collaborate with patient/family, physician, and IDG to deliver daily care.
• Instruct and supervise patient/family in self-care when appropriate.
• Maintain accurate clinical notes on patient condition.
• Functions effectively in home settings regardless of demographics.
