Job summary

Heart of Hospice, part of the LHC Group within Optum, seeks a Registered Nurse Case Manager to deliver hospice care at home. You will perform initial nursing evaluations, develop and implement plans of care, and coordinate with the interdisciplinary group to support patients and families facing life-limiting illness.

Requirements include active RN licensure in the state, a valid driver's license and reliable transportation; LA-specific experience and CPR certification are preferred.

Qualifications

• Current and unrestricted RN licensure in the state of practice.
• Current driver's license, valid vehicle insurance, and access to a dependable vehicle, or public transportation.
• LA-specific requirements: 2+ years of RN full-time experience; hospice experience may substitute for RN experience.

Responsibilities

• Make the initial nursing evaluation within 48 hours of referral to determine eligibility.
• Identify physical, psychosocial, and environmental needs; reassess at least every 15 days.
• Initiate and coordinate the plan of care; document goals, interventions, and responses.
• Collaborate with patient/family, attending physician and IDG to provide daily care.
• Instruct the patient/family in self-care and maintain accurate clinical notes.
• Function in diverse home environments and uphold equity in care.
