UnitedHealth Group in Boston is seeking an RN Case Manager to coordinate compassionate hospice care by assessing patients, partnering with the interdisciplinary team, and developing plans that meet each patient’s needs.

The role requires an active RN license in the state, CPR certification, a valid driver’s license, and at least one year of nursing experience. A $10,000 sign-on bonus is offered for external candidates, with a comprehensive benefits package and career growth opportunities.

Competenze

Current RN license valid for the state(s) you are assigned to work.
Valid driver’s license, reliable transportation and liability insurance.
Current CPR certification.
1+ years of nursing experience.

Mansioni

Performs patient assessments and collaborates with the care team to develop and implement a plan of care.
Makes referrals to other hospice care disciplines as needed or documents rationale for not doing so.
Develops, prepares, and maintains individualized patient care progress records with accuracy and timeliness.
Records pain/symptom management changes/outcomes as appropriate.
Teaches patients, families, and caregivers appropriate patient care techniques.
Regularly communicates with clinical manager and care team.
Performs on-call responsibilities and on-call services as assigned.
Other duties as assigned.
