Job summary

Graham Healthcare Group is seeking a Registered Nurse (RN) Hospice in Jacobs Creek, Pennsylvania to deliver compassionate end-of-life care in patients' homes. This staff position offers 40 hours per week, 8-hour shifts, and a robust bonus potential aligned with performance.

Qualified RNs must hold an active PA RN license, have at least two years of RN experience (hospice preferred), and a valid driver's license with travel ~20% of the time.

Qualifications

• Active RN license in Pennsylvania.
• Two years of RN experience; hospice experience preferred.
• Valid driver’s license and ability to spend ~20% of workday driving to patient locations.

Responsibilities

• Deliver compassionate, patient-centered hospice care in patients' homes.
• Perform comprehensive patient assessments and end-of-life care planning.
• Educate and support patients and families through the end-of-life journey.
• Ensure accurate EMR documentation and compliance.
• Travel to patient homes within assigned territory.
