Job summary

Graham Healthcare Group is seeking a Registered Nurse (RN) Hospice for a nursing job in Belle Vernon, Pennsylvania. The role covers home-based end-of-life care, with 40 hours per week and an 8-hour shift, traveling to patient homes within the territory.

You’ll work with an interdisciplinary team, perform assessments, manage symptoms, educate families, and document thoroughly in EMR. A valid PA RN license and at least two years of RN experience are required; hospice experience is preferred.

Qualifications

  • Active Registered Nurse with RN license in the state you work.
  • Two years of RN experience and clinical expertise to deliver quality patient care; Hospice experience preferred.
  • Valid driver’s license and ability to spend ~20% of workday driving to patient locations.

Responsibilities

  • Deliver compassionate, patient-centered care in a hospice setting and collaborate with an interdisciplinary team.
  • Perform comprehensive patient assessments, symptom and pain management, and end-of-life care planning.
  • Educate and support patients and families through the end-of-life journey and ensure informed decision-making.
