Qualifications

• Active RN license in the state you work.
• Two years of RN experience and hospice experience preferred.
• A valid driver’s license and the ability to travel approximately 20% of the workday.

Responsibilities

• Deliver compassionate, patient-centered hospice care at patients' homes.
• Perform comprehensive patient assessments, symptom and pain management, and end-of-life planning.
• Educate and support patients and families through the end-of-life journey with clear communication and empathy.
• Maintain thorough EMR documentation and collaborate with an interdisciplinary team.
