Job summary

Agape Hospice Care in Georgia is seeking a Registered Nurse to administer skilled nursing care for clients in their home and coordinate with the interdisciplinary team. The role requires an active RN license, CPR certification, and a minimum of two years of clinical RN experience, preferably in hospice or home health.

Strong interpersonal, communication, and organizational skills are essential. Proficiency with computers and MS Office is preferred.

Qualifications

• Graduate of an accredited Associate or bachelor’s degree in Nursing.
• Current state license as an RN and current state driver’s license.
• Two years’ experience as an RN in a clinical care setting; hospice or home health preferred.
• Current CPR certification.
• Proficient computer skills; Microsoft Office experience preferred.
• Strong interpersonal skills and ability to support patients and families.
• Demonstrate nursing practice skills and cope with family emotional stress.
• Strong communication and organizational skills; high ethics and compliance.

Responsibilities

• Administer skilled nursing care for clients in their place of residence.
• Coordinate care with the interdisciplinary team, patient/family and referring agency.
• Provide education about disease processes, end-of-life care, and self-care techniques.
• Initiate preventive and rehabilitative nursing procedures.
• Prepare clinical notes and progress updates aligned to goals.
• Coordinate patient/family services and prioritize needs with the IDT.
• Refer to other services as needed and use case management approaches.
• Inform physicians of changes and evaluate patient/family responses.
