Job summary

Agape Hospice Care in Georgia is seeking a registered nurse to administer skilled nursing care to patients in their homes, coordinate care with the interdisciplinary team, and support families throughout the hospice journey.

The role emphasizes comprehensive assessments, care planning, on-call responsibilities, and education for patients and caregivers. Two years of RN experience, CPR, and a valid license are required; hospice or home health experience preferred.

Qualifications

• Graduate of an accredited nursing program.
• Current RN license and valid driver’s license.
• Two years of RN experience; hospice or home health experience preferred.
• Current CPR certification.
• Proficient with computers; MS Office experience preferred.

Responsibilities

• Perform initial and ongoing assessments of the patient's condition and needs.
• Implement and update the plan of care (POC) based on assessment findings.
• Educate patients and families about disease process, self-care, end-of-life care, nutrition and caregiver support.
• Initiate preventive and rehabilitative nursing procedures as needed.
• Prepare clinical notes showing progress toward goals.
• Coordinate services with the interdisciplinary team and refer as needed.
• Communicate changes in patient needs to physicians and staff.
• Determine scope and frequency of services based on acuity and needs.
• Assess caregiver ability to meet patient needs and provide guidance.
