Job summary

Dependable Hospice is seeking an experienced Hospice Admissions RN to join our Tucson team. The role involves meeting with patients and families, completing assessments, and coordinating with the care team to ensure smooth admissions in home and community settings.

The ideal candidate will hold an Arizona RN license with at least 2 years of clinical nursing experience, plus familiarity with hospice care concepts.

Qualifications

• Must hold an Arizona Registered Nurse license.
• Must have at least 2 years of clinical nursing experience.
• Hospice, home health, palliative care, or community nursing experience is preferred.
• Knowledge of CMS guidelines and hospice eligibility is a plus.

Responsibilities

• Conduct comprehensive admission assessments in patients' homes and other care settings.
• Evaluate hospice eligibility and identify clinical needs.
• Educate patients and families about hospice services, benefits, and expectations.
• Coordinate with physicians, facilities, referral sources, and hospice team.
• Complete admission documentation accurately within required timeframes.
• Develop initial plans of care based on patient needs and goals.
• Provide education on symptom management, medications, equipment, and end-of-life care.
• Ensure medications, supplies, and equipment are arranged for admission.
• Offer compassionate emotional support to patients and families.
• Communicate critical information to the interdisciplinary hospice team.
• Maintain compliance with hospice policies and CMS requirements.
