Job summary

At Home Healthcare in Tucson, part of the LHC Group and the Optum family, seeks an experienced Registered Nurse in Home Health to deliver and coordinate nursing care in patients' homes as prescribed by physicians and policy. You will lead care plans, supervise aides, and ensure seamless communication with families, physicians, and team members.

The role requires current RN licensure in the state, a valid driver's license, and the ability to navigate the full scope of home health services.

Qualifications

• Current and unrestricted RN licensure in the state of practice.
• Current driver's license, vehicle insurance and access to a dependable vehicle or public transportation.

Responsibilities

• Initiates, develops, implements, and revises the plan of care in collaboration with the physician and other health care professionals.
• Supervises care provided by home health aides and licensed practical/vocational nurses, provides instruction, and assigns tasks according to State and federal regulations.
• Provides high-quality clinical services within the scope of practice and infection control standards, in accordance with the plan of care.
• Completes comprehensive assessments (OASIS) including medication reconciliation accurately and timely.
