Job summary

Three Oaks Hospice in Colorado is seeking a compassionate RN to administer skilled nursing care for patients at home in Boulder, Lafayette, and Louisville. This full-time role runs Monday through Friday and involves coordinating with the interdisciplinary team to ensure comfort, safety, and dignity at end of life.

You will conduct assessments, develop and revise the plan of care, educate patients and families about disease progression and symptom management, and provide on-call support as needed

Qualifications

• Graduate of an accredited RN program with license to practice.
• Two years RN experience in clinical care; hospice/home health preferred.
• Current CPR certification.
• Valid driver’s license.
• Proficient with computers; MS Office preferred.
• Strong interpersonal, communication, and organizational skills.

Responsibilities

• Perform initial and ongoing assessments of patient needs and eligibility for hospice care.
• Develop and update the plan of care with the IDT and patient-family.
• Educate patients and families about disease progression and care options.
• Coordinate services across disciplines and coordinate referrals.
• Provide on-call coverage for patients as assigned.
• Document progress notes and maintain care records.
