Job summary

Three Oaks Hospice is seeking a compassionate RN to join our Central/South Denver area team on a PRN basis. You will assess patients at home, coordinate with the interdisciplinary team, and deliver skilled nursing care in the patient’s residence.

Responsibilities include developing and updating the plan of care, teaching patients and families, on-call coverage as needed, and ensuring high-quality end-of-life care with empathy and professionalism.

Qualifications

• Graduate of an accredited Associate or bachelor’s degree in Nursing.
• Current state RN license and driver's license.
• Two years’ RN experience; hospice/home health preferred.
• Current CPR certification.
• Computer proficiency; MS Office experience preferred.
• Strong interpersonal, communication, and organizational skills.
• Ability to cope with family emotional stress and varied lifestyles.

Responsibilities

• Initial and ongoing assessments of patient needs and ADLs in the home.
• Implement and revise the plan of care with the IDT.
• Educate patients and families about end-of-life care and nutrition.
• Initiate preventive/rehabilitative nursing procedures.
• Prepare progress notes showing goals and outcomes.
• Coordinate services and prioritize needs with the IDT.
• Maintain case management and refer to additional services as needed.
• Inform physicians of changes in patient needs and outcomes.
• Determine scope and frequency of services based on acuity.
• Assess caregiver ability and provide support.
• Use available processes and technology; ensure admission/re-cert criteria.
• Perform on-call duties as assigned.
