Job summary

Three Oaks Hospice is seeking a compassionate Registered Nurse to provide skilled nursing care for patients in their homes across Boulder, Lafayette and Louisville regions. The role involves coordinating with the interdisciplinary team, planning personalized care, and supporting families through end-of-life care.

Ideal candidates hold an RN license in good standing, CPR certification, and experience in hospice or home health.

Qualifications

• Graduate of an accredited nursing program with RN license.
• Current CPR certification and driver's license.
• Experience in hospice or home health preferred.

Responsibilities

• Perform initial and ongoing comprehensive assessments of patient needs and ADLs.
• Coordinate care with the interdisciplinary team and care providers.
• Educate patients and families about disease processes, end-of-life care, and self-care.
• Initiate appropriate nursing procedures and ensure care plans are followed.
• Document progress notes and coordinate services across the care continuum.
• Communicate changes in patient status to physicians and team members.
