Job summary

Three Oaks Hospice, Inc. in Willis, TX is seeking an experienced Registered Nurse to administer skilled nursing care at patients' homes, coordinate care with the interdisciplinary team, and manage caseload within the coverage area of Conroe/Willis and surrounding communities.

The role requires an ADN/BSN, current TX RN license, valid driver’s license, CPR certification, and at least two years of RN experience (hospice/home health preferred).

Qualifications

• Graduate of an accredited Associate or bachelor’s degree in Nursing.
• Current state license as an RN and a valid driver's license.
• Two years’ RN experience in a clinical care setting; hospice/home health preferred.
• Current CPR certification.
• Proficient with computers; Microsoft Office experience preferred.
• Strong interpersonal, communication and organizational skills.
• Demonstrate ethical practice and professional integrity.

Responsibilities

• Initial and ongoing assessments of patient needs and ADLs.
• Implement the individualized plan of care and revise as necessary.
• Educate patient/family on disease, end-of-life care and self-care.
• Initiate preventive and rehabilitative nursing procedures.
• Prepare progress notes showing goal attainment.
• Coordinate services with the IDT and refer to needed resources.
• Inform physician of changes and document outcomes.
• Determine scope and frequency of services based on acuity.
• Assess caregiver ability to meet needs on admission and during care.
• Provide on-call coverage as assigned.
