Job summary

Three Oaks Hospice, Inc. is seeking an experienced RN to provide skilled nursing care for patients in their homes in the Louisville area. You will coordinate with the interdisciplinary team, manage plan of care, and educate families on end-of-life care.

Ideal candidates hold an active RN license, CPR certification, and at least two years of clinical RN experience (hospice/home health preferred). A valid driver’s license and computer proficiency are required to support patient care and

Qualifications

• Graduate of an accredited Associate or bachelor’s degree in Nursing.
• Current state RN license and valid driver’s license.
• Two years’ RN experience in a clinical care setting; hospice/home health preferred.
• Current CPR certification.
• Proficient using a computer; MS Office experience preferred.
• Strong interpersonal and organizational skills.

Responsibilities

• Conduct initial and ongoing assessments of patient needs and ADLs.
• Implement the individualized plan of care and revise as needed.
• Educate patient/family about disease, self-care, and end-of-life care; train staff as needed.
• Initiate preventive and rehabilitative nursing procedures.
• Prepare clinical and progress notes showing goal achievement.
• Coordinate patient/family services with the IDT and referring agencies.
• Utilize case management and refer to other services as needed.
• Inform physician of changes in patient needs and responses to care.
• Determine scope and frequency of services based on acuity.
• Assess caregiver ability to meet patient needs; provide support.
• Communicate using current organization technology and processes.
• Apply admission and re-certification criteria for hospice care.
• Perform on-call duties and provide on-call service as assigned.
• Other duties as assigned.
