Qualifications

• Must be a Registered Nurse licensed to practice in North Carolina.
• Minimum of one year of professional nursing experience in an acute care setting preferred.
• Demonstrated leadership abilities and strong communication with patients, families, staff, and physicians.

Responsibilities

• Coordinate hospice services per assigned counties and agency policies.
• Maintain accurate electronic documentation in EMR and ensure timely updates.
• Collaborate with physicians and the IDG team to update plans of care.
• Supervise LPNs and paraprofessionals, ensuring CMS/Hospice guidelines are followed.
• Provide on-call coverage as required and support the interdisciplinary team.
