Job summary

Qualifications

• RN license in North Carolina (NC)
• Minimum of one year as a professional nurse in acute care preferred
• Evidence of leadership qualities
• Experience with terminally ill patients preferred
• Ability to understand and communicate well with patients, families, staff, physicians, and the public
• CPR (BCLS) certification currently maintained
• Dependable and able to interact tactfully with patients, families, staff, providers, and the public
• Competent to perform delegated tasks to support patient outcomes as per care plan
• Valid Driver's License and reliable transportation

Responsibilities

• Abides by and supports 3HC's Compliance Program and Code of Ethics; promotes ethical and legal practices
• Delivers positive experiences to patients and internal/external customers with courtesy and knowledge
• Provides skilled nursing care per physician's plan of care and coordinates hospice delivery
• Completes hospice admissions and educates patients/families on hospice benefits
• Plans and directs delivery of compassionate, high-quality patient care and participates in interdisciplinary assessments
• Maintains electronic documentation in EMR within 48 hours
• Communicates with physicians and Hospice Medical Director about patient changes and plan modifications
• Supervises LPNs and paraprofessionals per CMS/Hospice guidelines; conducts regular supervision and evaluations
• Collaborates with supervisor to meet productivity standards; participates in on-call rotations
• Rotates weekday/weekend/holiday on-call; covers visits at other offices as assigned and supports referral processes
• Maintains regular communication with supervisor and coordinates hospice care using current processes and technology
• Ensures timely certifications and re-certifications for billing; keeps up with reimbursement criteria
