Job summary

Nova Hospice is seeking an experienced Hospice Registered Nurse to plan, organize and direct care in patients' homes and communities across Texas. The RN will lead nursing assessments, coordinate care with an interdisciplinary team, and support families with education and planning.

The role requires current Texas RN licensure, two years of nursing experience (preferably in home care or hospice), CPR certification, and a valid driver's license.

Qualifications

• Graduate of NLN-accredited nursing program.
• Minimum of two years nursing experience, preferably in public health, home care or hospice.
• Current Texas RN license.
• CPR certification maintained.
• Strong communication and ability to work independently.
• Valid driver’s license with insured vehicle.

Responsibilities

• Performs comprehensive patient and family assessments to determine hospice needs.
• Provides professional nursing care using the nursing process.
• Develops and revises care plans with the interdisciplinary team.
• Administers medications and treatments as prescribed by physician.
• Educates patients and families on self-care and disease management.
• Acts as Case Manager, coordinating care for assigned caseload.
• Maintains accurate clinical notes and communicates with physicians.
• Participates in on-call duties and coordinates equipment needs.
