Job summary
MI CASA HEALTHCARE LLC in San Antonio, TX is seeking a full-time Hospice Registered Nurse (RN) to provide compassionate end-of-life care in homes and community settings. The role emphasizes dignity, comfort, and collaboration with the interdisciplinary team.
The ideal candidate holds a Texas RN license, with at least 2 years of nursing experience and 1 year in hospice care; BLS/CPR is required and a valid driver's license is necessary for on-site visits.
Qualifications
- Current Texas RN license in good standing.
- Minimum 2 years nursing experience; hospice experience preferred.
- BWLS/CPR certification; valid driver’s license and reliable transportation.
Responsibilities
- Provide skilled hospice nursing care in homes, facilities, and community settings.
- Participate in an on-call rotation with biweekly IDG meetings.
- Develop and implement individualized care plans with the interdisciplinary team.
- Administer medications and monitor patient comfort and safety.
- Document clinical notes within 24 hours of visits; ensure accuracy.
- Educate patients and families on hospice services and end-of-life care options.
- Maintain compliance with Medicare, state regulations and company policies.
