Job summary

Frontpoint Health seeks an experienced Registered Nurse to provide in-home hospice care across patient ages in the Richmond, Texas area. You will coordinate with the Interdisciplinary Group/Team, patients and families to plan and deliver comprehensive nursing care, ensuring continuity and quality of service.

The role requires holding an RN license, at least two years of clinical RN experience, and strong communication skills.

Qualifications

• Graduate from an accredited Diploma, Associate, or Baccalaureate School of Nursing.
• Current state license as a Registered Nurse (RN).
• Two years of experience as an RN in a clinical care setting; hospice experience required.
• Nursing skills as defined by generally accepted standards of practice; good interpersonal skills.

Responsibilities

• Perform initial and ongoing comprehensive assessments of patients in their home.
• Apply admission and recertification criteria for hospice care.
• Develop and revise care plans to ensure quality and continuity of care.
