Job summary

ViaQuest Hospice in Indiana seeks a compassionate Registered Nurse to coordinate patient care in hospice. You will manage a primary caseload, assess needs, plan, implement and evaluate nursing interventions, and collaborate with physicians and the care team.

The role includes administering medications, maintaining records, supervising LPNs and aides, and traveling across the service area with a flexible schedule.

Qualifications

• Associate degree or equivalent in Nursing from an accredited program.
• RN license in good standing.
• Hospice experience preferred.
• Valid driver's license and reliable transportation.
• Willingness to travel throughout assigned service area.

Responsibilities

• Provide physical and emotional support to patients and families.
• Assume primary responsibility for a patient and family caseload including assessments, planning, implementing and evaluating phases of the nursing process.
• Administer and instruct the administration of medication.
• Initiate communication with physicians, other hospice team members and other agencies to coordinate care.
• Maintain current patient records and progress notes.
• Supervise LPNs and Hospice Aides and document delivery of care as required.
