Job summary

ViaQuest Hospice in Linton, IN seeks a compassionate CNA to provide direct patient care within a hospice setting. You will perform tasks per the plan of care and communicate patient status updates to the supervising RN.

A valid CNA certification and willingness to travel the service area are required. Responsibilities include maintaining safety, providing emotional support to patients and families, and collaborating with an interdisciplinary team.

Qualifications

• Current CNA certification.
• At least 18 years of age.
• High school diploma or GED.
• Valid driver's license, car insurance and reliable transportation.
• Willingness to travel throughout assigned service area.
• Can push, lift or carry up to 50 lbs. and can climb, balance, kneel and crouch.

Responsibilities

• Performing duties as outlined in the plan of care.
• Effectively communicating updates and changes to patient's status to the supervising RN.
• Maintaining a safe patient environment.
• Providing emotional support and compassionate care to patients and families.
• Maintaining current patient records and progress notes.
• Working as a member of the interdisciplinary team.
