Job summary

ViaQuest Hospice is seeking a compassionate Certified Nursing Assistant to provide holistic care to patients in their homes and communities. You will follow the plan of care, communicate patient status to the supervising RN, and maintain a safe, supportive environment for patients and families.

You will contribute to an interdisciplinary team, maintain accurate records, and travel within the service area. Comprehensive training and a full benefits package accompany this role.

Qualifications

• Current CNA certification.
• At least 18 years of age.
• High school diploma or GED.
• Valid driver's license, car insurance and reliable transportation.
• Willingness to travel throughout assigned service area.
• Can push, lift or carry up to 50 lbs. and can climb, balance, kneel and crouch.

Responsibilities

• Performing duties as outlined in the plan of care.
• Effectively communicating updates and changes to patient's status to the supervising RN.
• Maintaining a safe patient environment.
• Providing emotional support and compassionate care to patients and families.
• Maintaining current patient records and progress notes.
• Working as a member of the interdisciplinary team.
