Caris Healthcare is seeking a Registered Nurse, RN to provide hospice/home health care under physician orders. You will assess, plan, implement, and evaluate total patient care and nursing care plans while collaborating with interdisciplinary teams.

The role emphasizes compassionate patient advocacy, education, and adherence to policies. Eligible candidates must be licensed as a RN in the operating state; experience in hospice or home health is preferred.

Qualifications

• Must be a Registered Nurse licensed in the state of operation.
• Nursing experience with hospice or home health preferred.
• Solid knowledge of nursing principles and patient teaching.

Responsibilities

• Assess, monitor, and document patient symptoms using required software.
• Collaborate with patient, family, partners, and interdisciplinary team to develop care plans.
• Advocate for patients, explain rights and procedures, and support families.
• Monitor patient status and educate caregivers; supervise aides as needed.
• Participate in on-call schedule and provide back-up assistance.
